Dr. Ayesha Kanwal, DO

Head of Care Management Programs 

Dr. Kanwal is a board-certified internist and fellowship-trained rheumatologist with more than sixteen years of clinical experience. She leads clinical oversight of CareNexa’s chronic care management, remote monitoring and principal care management programs.

She earned her Doctor of Osteopathic Medicine at the University of North Texas Health Science Center, completed her internal medicine residency at MetroHealth Medical Center and her rheumatology fellowship at University Hospitals Cleveland, with externships at Cleveland Clinic Weston Hospital and Henry Ford Hospital. She holds a bachelor’s degree from the University of Texas at Arlington. 

Alongside her work with CareNexa, Dr. Kanwal is the founder and medical director of Wellness Rheumatology in Colleyville, Texas, where she continues to see patients. She has served as an Associate Professor at the Texas Christian University School of Medicine for the past four years, teaching medical students and residents from TCU, the University of North Texas and Medical City.

Why a rheumatologist leads a chronic care program 

Rheumatology is a specialty defined by long visit intervals and heavy between-visit management. Patients on immunosuppressive therapy need laboratory monitoring on schedule, infection surveillance, and someone tracking whether a biologic reached them this month. A flare in week six of a twelve-week interval is managed by the patient, alone, unless somebody is in contact.

That is the same problem chronic care management exists to solve, in a specialty where the consequences of getting it wrong are unusually visible. Dr. Kanwal has spent sixteen years working inside it.
